Clinical Case Manager, RAFA

The Real Alternatives for Adolescents (RAFA) Clinical Case Manager position is an ideal position for a recent MSW graduate who is looking to gain clinical experience and earn clinical supervision and group supervision hours toward licensure. RAFA provides housing and case management to foster youth or youth who are currently on probation. Youth learn independent living skills with a safety net and are supported in meeting their academic, employment and relationship goals. 

The RAFA Clinical Case Manager (CCM) provides clinical care and intensive case management to transition age youth (16-21) Youth live independently in apartments with a roommate and regular assistance provided by a support team. The RAFA Clinical Case Manager is part of a multidisciplinary team that provides community based mental health services, with an emphasis on skill building and case management.  The RAFA Clinical Case Manager actively supports and implements a strengths-based, culturally competent, client-centered approach to working with TAY. This exempt position may require work weeks in excess of 40 hours as well as evening and weekend work.


  • Manage a caseload of 10-13 youth shared with the Youth Development Specialist to implement a strengths based individual plan as part of a multidisciplinary team
  • Partner with Youth from intake and discharge process, completing strength-based mental health and needs assessments.  Meeting regularly to identify and working on short and long term mental health and independent living skills goals.
  • Partner with youth in involving family and other informal and formal key players in planning services and supports for each youth.  Help youth maintain and strengthen relationships with permanent support people.
  • Partner with youth in identifying obstacles to living independently, obtaining employment, obtaining post-secondary education, and in developing supportive social and recreational activities. Develop strategies to overcome these barriers.
  • Participating in external and in-services trainings relevant to the program services.
  • Timely completion of documentation of services in accordance with Medi-Cal recording standards as set out in program policy.


  • Masters degree in Social Work (MSW) from an accredited university or college.
  • Registered as an associate clinical social worker (ACSW) with with California BBS required
  • At least 2 years post-graduate experience working with adolescents, preferably currently and/or formerly foster/probation TAY
  • Fulfillment of TB clearance, fingerprinting and state clearance, and any other mandatory state/federal requirements.
  • A valid California driver's license, personal automobile for use on the job, insurable by SHS’ insurance policy based on DMV record and personal vehicle insurance.
